Eligibility. You must be at least 18 years old and able to enter into a legally binding agreement to use our services.
Services Provided. Autumn ABA provides Applied Behavior Analysis (ABA) therapy to children with autism and related developmental disabilities. Services are delivered in-home, in daycare, school, and community settings, or via telehealth when authorized.
Availability. All services are subject to therapist availability, insurance authorization, and completion of intake forms.
Prohibited Use. You agree not to use our services in any unlawful manner, or in a way that disrupts operations, breaches confidentiality, or misrepresents your eligibility.
Accurate Information. You agree to provide accurate, current information during intake and throughout care, including insurance, contact, and address information.
Timely Communication. We rely on timely responses from families to ensure effective scheduling and care coordination.
Parental Involvement. Parents/guardians must be available for communication and participate in parent training when part of the treatment plan.
Insurance Coverage. We accept Medicaid and most major insurance plans. Coverage is verified before starting services, but we cannot guarantee approval or payment by your insurance.
Financial Responsibility. You are responsible for any copayments, deductibles, or out-of-network fees not covered by your insurance provider.
Private Pay Clients. Clients without insurance coverage will be provided a private pay agreement with fee terms.
Cancellations. You must notify us at least 24 hours in advance for cancellations. Repeated or late cancellations may lead to service interruptions or fees.
By providing your mobile number, you consent to receive text messages from Autumn ABA regarding your services and scheduling.
Opt-Out. Reply โSTOPโ to any message to unsubscribe. You will receive a confirmation and will no longer receive texts.
Essential Messages. We may continue sending essential service messages, even if you opt out of promotional communications.
Re-Enrollment. To resume messages, reply โSTARTโ or contact our office.
